Payment Policy

The fee charged for our professional service is $150.00 per clinical hour.  A clinical hour is established by current CPT codes as designated by the American Medical Association and is between 38 and 52 minutes.  Additional time over 52 minutes is adjusted according to the CPT code. The fee for initial assessment is $200.00.  The fee amount for each session is due and must be paid at the conclusion of each session.  There are exceptions with third party payments or any assignment of benefits.  These exceptions are made known prior to treatment and are at the discretion of SMS Office Services.  Upon approval, copayments are due prior to each session.  A monthly statement of your account will be provided by the tenth of the following month for services rendered.  Some health insurance companies will reimburse patients for services and some will not.  A diagnosis will be required before reimbursement is made.  I will provide the patient with the diagnosis based on the evaluation and assessment. This diagnosis will become part of the patient's permanent insurance record. Please contact your insurance provider to clarify any personal questions. All questions regarding billings and payments are to be discussed with SMS Office Services. In the event you are not able to keep your appointment, you will be responsible for paying for the missed session.  There are exceptions with emergeant situations. Telephone contact to discuss therapeutic issues is prorated at the hourly clinical rate and is billed accordingly.

Payment Inquiries

What if I do not understand my diagnosis?

Contact SMS Office Services at 304-671-1313 to discuss your inquiry with Susan M. Poore.  She will provide you with billing information. If you are not satisfied, you will be directed to discuss such with your therapist.



What if I cannot make my appointment?

You are still responsible for the assessed fee. There are exceptions in emergency situations. The reason for your absence is to be discussed with SMS Office Services and a decision will be made at the time of your inquiry.

What if I do not know if my insurance benefit will cover services?

Once you provide your insurance information to Susan M. Poore at SMS Office Services, she will make all the necessary arrangements with your insurance provider. Ms. Poore will inform you of your benefit and your obligation for payment.

What will be the duration of my treatment?

This will need to be  discussed with your therapist. Treatment can take a few sessions or can continue for years. It is dependent on the diagnosis, comorbidity, and response to treatment.
